Responsibilities

• Provides personal care to patients according to the assignment details and care plan.
• Documents patient care and observations timely and appropriately.
• Participates in end-of-life care for patients and families.
• Informs the registered nurse of any changes in the patient's condition.
• Follows the schedule of care, including making several visits per day with travel.
• Assists patients with activities of daily living, such as bathing, dressing, and grooming.
• Monitors and records vital signs, including temperature, pulse, and respiration.
• Supports patients with mobility and exercise as prescribed.

Requirements

• Education: High School Graduate or Equivalent (required)
• Work Experience: Experience in the health care field (required)
• Hospice experience (preferred)
• Hospice Aide training from an approved 75‑hour training course (required)
• Licenses/Certifications: Basic Life Support – CPR (BLS) (required), Driver's License (DL) (required), Certified Nurse Assistant (CNA) (required)
