About the role
The Hospice Aide (HA) is responsible for providing direct care to hospice and long-term care patients in their primary place of residence. The role involves improving quality of life and promoting comfort according to the Hospice plan of care. Tasks include bathing, dressing, nail care, skin care, assisting with elimination, nutritional needs, mobility transfers, homemaking, maintaining a clean and safe environment, taking vital signs, and observing changes in patient conditions.
Responsibilities
- Deliver quality patient care following the HA plan of care
- Perform tasks such as bathing, dressing, nail care, skin care, assist with elimination, nutritional needs, mobility transfers, homemaking, maintaining a clean safe environment, and taking vital signs
- Observe changes in patient conditions and document accordingly
- Recognize and respond appropriately to emergencies
- Provide care and support to patients and families during times of crisis and end of life
- Work within the scope of practice for Hospice Aide under the supervision of the Hospice Registered Nurse
- Follow instructions provided verbally, in writing, and in the EMR mobile device
- Carry out duties and responsibilities of position in a manner that exemplifies excellent customer service
- Represent Keystone Hospice to the community in a positive manner
- Adhere to Agency, State, and Federal policies and procedures, laws, and regulations relevant to job responsibilities
- Ensure compliance with Medicare Hospice & Long-Term Care regulations including HIPAA Privacy Standards and related Agency policies
- Must be fully vaccinated against Covid 19, or obtain a religious or medical exemption upon hire
Requirements
- Certified by the state as a Certified Nursing Assistant (CNA), preferred or qualified HHA
- Successful completion of Home Health/Hospice Aide competency evaluation requirements required to perform duties according to state and federal requirements (Medicare Conditions of Participation)
- CPR and First Aid certification or the ability to obtain certification before starting employment
- Reliable transportation, current State driver's license, and valid vehicle insurance
Qualifications
- Minimum of one year of CNA/HHA/HA experience preferred, but not required; hospice/home health/long-term care experience preferred
- Travel between long-term care facilities and/or place of primary residence
- Attend bi-weekly IDT meetings, if requested
