Make Every Moment Matter as a Hospice Aide (CNA/HHA)! Are you a caring and dedicated Certified Home Health Aide (CNA/HHA) looking to make a meaningful impact? Continuum Hospice is seeking a Full‑time Hospice Aide (CNA/HHA) to join our compassionate team, providing hands‑on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Schedule

Sunday - Friday, 8:00a - 4:30p. Coverage Area: Sonoma & Marin Counties.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time.
• Maintain Accurate Documentation: Every detail is crucial in delivering high‑quality care, both in the patient's home and within our agency and regulatory standards. Your attention to detail is vital to our success.

Qualifications

• High School Diploma or G.E.D. required.
• Valid Certified Home Health Aide (HHA) license.
• Valid Certified Nursing Assistant (CNA) license required.
• Minimum of 6 months experience in hospice or skilled nursing facility preferred.
• Hospice experience a plus!
• Possess and maintain current CPR certification if state‑mandated.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Pay

$27.00 - $29.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
