Hospice Aide

About the role

Concord Hospice is a comprehensive and compassionate care option for individuals going through the final stages of life. We care for terminally ill patients at their place of residence, whether that is at home, a skilled nursing facility, or another residential setting. Hospice focuses on delivering comfort to terminally ill patients after attempts to cure them have failed. Our services are designed to meet the physical, emotional, spiritual, and practical needs of patients and their loved ones. Above all, it’s the comfort and well-being of our patients that drive all that we do.

Responsibilities

• Provide direct patient care including administering medications, assisting with hygiene, and performing other tasks as needed.
• Assist patients and families with daily living activities such as bathing, dressing, and eating.
• Document patient care in accordance with hospice policies and procedures.
• Communicate effectively with patients, families, and other healthcare providers to ensure continuity of care.
• Ensure the cleanliness and safety of the patient’s living environment.

Requirements

• High school diploma preferred.
• Current Certified Nursing Assistant (C.N.A.) certification required.
• One year of experience as a hospice aide preferred.
• Current driver license required.
