About the role
VIA Health Partners is seeking dedicated individuals to join our team. We are a community-based, not-for-profit hospice and palliative care provider committed to serving all patients and families regardless of their ability to pay or medical complexity.
Responsibilities
- Assumes responsibility for the effective coordination of patient/caregiver care.
- Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
- Visits patients to provide appropriate interventions, following the established plan of care and evaluating patient/caregiver responses to determine progress toward goals.
- Develops, implements, and evaluates plans of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions and addressing patient/caregiver education needs.
- Evaluates and updates plan of care as goals and objectives are achieved or changed.
- Emergency triage by telephone and home visits.
- Provides interventions consistent with the plan of care and performs all interventions required for each visit, identifying new problems or needs when they occur.
- Facilitates patient and caregiver independence to the extent possible.
- Facilitates communication between the patient, caregivers, physicians, facility staff, and interdisciplinary team.
- Identify and refer all patient/caregiver psychosocial needs to VIA Health Partners Medical Social Worker as appropriate.
- Identify learning needs of patients and caregivers and provide necessary instruction.
- Demonstrate nursing care procedures, utilizing teaching materials, tools, and written information appropriately.
- Encourage and allow time for questions and consultations.
- When providing hospice care, appropriately utilize VIA Health Partners policies, patient-related protocols, standing orders, and guidelines, including Enclara Pharmacia MUGs; Medline, and ProHospice, as applicable.
- Maintain productive and professional relations with external agencies and healthcare providers.
- Store all supplies safely and ensure that work area is clean, secure, and well-maintained.
- Maintain appropriate utilization of resources to ensure cost-effective delivery of healthcare.
- Utilize principles of infection control and ensure universal precautions in accordance with accepted professional practice.
- Keep clinical records current by synchronizing laptop information throughout the shift.
- Complete and submit all clinical documentation per VIA Health Partners clinical documentation guidelines.
- Maintain current medication profile as directed by physician.
- Document medication teaching: purpose, doses, route, frequency, actions, indication for use, side effects, and interactions.
- Record time/mileage accurately.
Qualifications
- A Diploma or Associates Degree in nursing is required; a Bachelor’s Degree is preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- CPR certification and CEUs are required.
- Minimum 2 years related experience, preferably in direct care of patients with life-limiting illnesses, including hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical.
- Prior experience in working in an environment focused on the pediatric population is required for Kids Path™ Nurse with BCLS (CPR) training for both adult and child/infant.
- Computer proficiency is required.
