About the role

VIA Health Partners is seeking dedicated individuals to join our team. We are a community-based, not-for-profit hospice and palliative care provider committed to serving all patients and families regardless of their ability to pay or medical complexity.

Responsibilities

  • Assumes responsibility for the effective coordination of patient/caregiver care.
  • Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
  • Visits patients to provide appropriate interventions, following the established plan of care and evaluating patient/caregiver responses to determine progress toward goals.
  • Develops, implements, and evaluates plans of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions and addressing patient/caregiver education needs.
  • Evaluates and updates plan of care as goals and objectives are achieved or changed.
  • Emergency triage by telephone and home visits.
  • Provides interventions consistent with the plan of care and performs all interventions required for each visit, identifying new problems or needs when they occur.
  • Facilitates patient and caregiver independence to the extent possible.
  • Facilitates communication between the patient, caregivers, physicians, facility staff, and interdisciplinary team.
  • Identify and refer all patient/caregiver psychosocial needs to VIA Health Partners Medical Social Worker as appropriate.
  • Identify learning needs of patients and caregivers and provide necessary instruction.
  • Demonstrate nursing care procedures, utilizing teaching materials, tools, and written information appropriately.
  • Encourage and allow time for questions and consultations.
  • When providing hospice care, appropriately utilize VIA Health Partners policies, patient-related protocols, standing orders, and guidelines, including Enclara Pharmacia MUGs; Medline, and ProHospice, as applicable.
  • Maintain productive and professional relations with external agencies and healthcare providers.
  • Store all supplies safely and ensure that work area is clean, secure, and well-maintained.
  • Maintain appropriate utilization of resources to ensure cost-effective delivery of healthcare.
  • Utilize principles of infection control and ensure universal precautions in accordance with accepted professional practice.
  • Keep clinical records current by synchronizing laptop information throughout the shift.
  • Complete and submit all clinical documentation per VIA Health Partners clinical documentation guidelines.
  • Maintain current medication profile as directed by physician.
  • Document medication teaching: purpose, doses, route, frequency, actions, indication for use, side effects, and interactions.
  • Record time/mileage accurately.

Qualifications

  • A Diploma or Associates Degree in nursing is required; a Bachelor’s Degree is preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • CPR certification and CEUs are required.
  • Minimum 2 years related experience, preferably in direct care of patients with life-limiting illnesses, including hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical.
  • Prior experience in working in an environment focused on the pediatric population is required for Kids Path™ Nurse with BCLS (CPR) training for both adult and child/infant.
  • Computer proficiency is required.
