About the role

The Hospice Admissions Nurse (Start of Care SOC Visit RN) will initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician, providing individualized, in‑home coordinated hospice care to bring safety and comfort to patients and families.

Responsibilities

• Complete an initial/comprehensive assessment of the patient/family to determine hospice needs, including history of current and previous illnesses and a physical assessment with emphasis on pain and symptom management.
• Complete SOC visits and all required documentation and tasks.
