About the Role
Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in adult and geriatric patients within their homes. Develop individualized treatment plans, deliver evidence-based interventions, educate patients and caregivers, and collaborate with an interdisciplinary team. Document care in the EMR, contribute to quality improvement initiatives, and support community-focused healthcare delivery.
