Home Visitation Family Support Case Manager

About the role

The Child and Family Guidance Center, founded in 1925, provides culturally informed mental health care and complementary supports, free of charge to those in need. We seek a full-time Home Visitation Program Family Support Specialist to join our team.

Responsibilities

• Provide comprehensive and intensive home visiting services to both parents using the Healthy Families America's evidence-based model to improve parenting skills, enhance the family’s support system, and improve parents' understanding of children’s developmental stages.
• Develop a strength-based "Action Plan" to address areas identified as challenges with the family, assist in setting and achieving goals, and assess the normal growth and development of the target child.
• Aid families in addressing barriers to attending important appointments.
• Maintain accurate records of all client contacts and records using multiple systems.
• Assist and model for the parents how to access resources and utilize their natural supports.
• Maintain collaborative relationships with other services and providers.

Requirements

• Bachelor’s or Associate’s degree in Early Childhood with training as a Parent Educator.
• Minimum two years’ experience working with young children and families, prenatal to age six.
• Previous experience working with vulnerable families.
• Excellent analytical, interpersonal, and communication skills (verbal and written).
• Computer proficiency.
• Ability to communicate effectively with people from diverse social and ethnic backgrounds.
• Knowledge of child abuse and neglect (mandated reporting) statutes preferred.

Qualifications

• Fully vaccinated for COVID-19 virus, unless eligible for a medical or religious exemption.
• Bilingual Spanish (preferred).
