Home Infusion Pharmacist

About the Role

This position is responsible for all facets of pharmaceutical care related to intravenous homecare patients. Responsibilities include assisting with the referral process, providing medical information to patients and caregivers about home infusion therapy, performing patient assessments, and overseeing the Patient Advocate team.

Responsibilities

• Compounds sterile medications and infusions for home, outpatient clinics, skilled nursing facilities, and physician/clinic environments.
• Initiates infusion therapies with ongoing evaluation of care and completes required documentation for processing therapies, both fiscal and legal.
• Assists in teaching patients prior to discharge to home.
• Coordinates deliveries with nursing and support services.
• Acts as Pharmacist in Charge (PIC) in the absence of the PIC, making decisions while on duty.
• Participates in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance, and hazardous waste control.
• Performs emergency on-call duties on weekends and weeknights as required by center scheduling.
• Adapts behavior to specific patient populations, including respect for privacy, communication style, and explanation of services or procedures.
• Remains knowledgeable on current federal, state, and local laws, accreditation standards, or regulatory requirements applicable to the assigned area.
• Performs other related duties as assigned.

Requirements

• Bachelor of Science in Pharmacy (required).
• Doctor of Pharmacy (PharmD) (preferred).
• Completion of a PGY1 or PGY2 Pharmacy Practice Residency (preferred).
• License to practice Pharmacy in the state of practice (required).
• 2 years of experience as a Pharmacist (preferred).
• Experience in specialty pharmacy (preferred).

Skills

• Computer skills and dexterity for data entry and information retrieval.
• Effective verbal and written communication skills.
• Proficiency with Windows-style applications, various software packages, and keyboard use.
• Strong interpersonal skills.
• Ability to use computer programs for review, authorization, preparation, dispensing, and management of home infusion center patients.
• Familiarity with literature, references, and resources for therapies and care of home infusion patients.
• Ability to maintain patient confidentiality.
• Ability to foster an environment of teamwork and engagement.

Physical and Environmental Demands

• Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
• Physical demand requirements exceed those for Sedentary Work, including walking or standing to a significant degree.
• Normal routine involves no exposure to blood, body fluid, or tissue, and no requirement to perform or assist in emergency care or first aid.
• No occupational risk for exposure to communicable diseases, but potential risk for exposure to hazardous medications or waste within the healthcare environment.
