About the role

As a Home Health Occupational Therapist, you will provide therapeutic services to patients in their homes or other settings, helping to improve or restore their daily living and work-related skills.

Responsibilities

  • Assess and screen patients' daily living and work-related skills, then develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to assist physicians in evaluating patients' levels of function.
  • Confer with physicians and other healthcare team members to develop or revise the Plan of Care Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range-of-motion, and function, or teach compensation measures.
  • Review and expand retraining programs consistent with each patient's behavioral goals and within rehabilitation program guidelines.
  • Consult with vocational team members to develop and implement vocationally oriented plans tailored to patients' needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, clinical managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, acting as a resource for other healthcare personnel.
  • Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
  • Recommend or design special adaptive equipment to improve clients' residence or working environments or enhance their participation in the rehabilitation program.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations on the day services are rendered.
  • Ensure interim (verbal) orders from physicians are accurately documented and implemented.
  • Submit evaluation, treatment plans, and discharge summaries to supervisors and care management staff.

Requirements

  • Degree from an accredited Occupational Therapy Program.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.

Qualifications

  • A minimum of six months of occupational therapy experience is preferred.
  • Home Health experience is a plus.
