About the role

As a Speech-Language Pathologist (SLP) in home health, you will assess, evaluate, and provide professional services prescribed by the physician to homebound patients. You will assist the physician in evaluating patients to determine the type of speech or language disorder and the appropriate corrective therapy, then develop and implement rehabilitation plans in consultation with the physician.

Responsibilities

• Assess and evaluate patients to determine speech or language disorders and appropriate corrective therapy.
• Develop rehabilitation plans based on evaluation and goals established in consultation with the physician.
• Provide rehabilitation services for speech and language disorders.
• Complete evaluations, technical treatments, and restorative procedures common in visit-based speech therapy practice.
• Observe and record the patient’s reaction to treatment and changes in the patient’s condition; notify physician and appropriate health professionals of any changes.
• Maintain confidentiality of patient assessments and medical records.
• Communicate effectively with patients, families, and members of the healthcare team.
• Attend Case Conference and make referrals to other disciplines as indicated by patient needs.
• Adhere to standard precautions, transmission precautions, aseptic techniques, and infection control guidelines.
• Instruct healthcare team members and families in methods to assist the patient in improving communication related to speech and language.
• Regularly communicate with RN/Therapist and meet with the Director of Patient Services.
• Attend required in-services and training.

Requirements

• Completion of a speech-language pathology curriculum in accordance with the state Speech-Language and Hearing Practice Act.
• Current state license to practice as a speech-language pathologist.
• Current CPR certification.
• Auto insurance and valid state driver’s license.
• Demonstrated professionalism in appearance, performance, and communication.
• Ability to provide safe, competent care to patients of all ages while respecting patient rights.
