Home Health Speech Language Pathologist - PRN

About the Role

This position involves providing personalized home health speech/language pathology services to patients, focusing on rehabilitation, functional improvement, and enhancing quality of life.

Responsibilities

• Evaluate, develop, and review treatment plans
• Assess and treat speech, voice, language, hearing, and swallowing disorders
• Administer diagnostic tests
• Monitor patient progress
• Instruct patients on communication aids
• Provide counseling and education to patients, families, and healthcare staff
• Maintain accurate documentation
• Participate in care coordination and discharge planning
• Ensure confidentiality of patient information

Requirements

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA)
• Minimum six months of experience
• Valid state license
• CPR certification
• Reliable transportation and a valid driver’s license

Preferred Experience

• Home health experience

Additional Details

• Compensation is based on visit/unit rate
• TB screening required
• Adherence to driver safety policies
