About the role

As a Home Health Speech‑Language Pathologist at CenterWell Home Health, you will report to the Clinical Manager and provide personalized rehabilitation services in patients’ homes or facilities, helping them regain strength, mobility, independence, and quality of life.

Responsibilities

  • Evaluate, direct, and provide speech‑language pathology services to patients in the home or facility.
  • Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
  • Assess and treat disorders of speech, voice, language, hearing, and swallowing, using professional judgment to prevent, identify, evaluate, and minimize their effects.
  • Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening.
  • Observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician.
  • Provide instruction and training to patients on alternative communication systems when appropriate.
  • Counsel and instruct patients, families, and healthcare staff.
  • Maintain and submit documentation required by the company and/or facility, including written reports of evaluations, visits, summaries, care plans, coordination activities, and progress reports.
  • Participate in care coordination activities and discharge planning.
  • Uphold the highest standards of professional conduct regarding confidential information, sharing it only when appropriate and in the patient’s best interest.

Required qualifications

  • Certification of Clinical Competence in Speech‑Language Pathology or Audiology granted by ASHA.
  • Minimum six months experience as a speech therapist / speech‑language pathologist.
  • Current, unrestricted license.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.

Preferred qualifications

  • Home health experience.

Pay

  • Pay per visit/unit rate: $54.00 – $76.00.
  • Estimated full‑time base salary range: $85,400 – $117,500 per year (based on location, experience, and other factors).

Schedule

  • Full‑time position, approximately 40 hours per week.

Additional information

  • TB screening: This patient‑facing role participates in Humana’s Tuberculosis screening program; selected candidates must be screened for TB.
  • Driving requirements: Must maintain a valid state driver’s license and personal vehicle liability insurance meeting state minimum limits or $25,000 bodily injury per person / $25,000 per event / $10,000 property damage, whichever is higher.
