Home Health Speech Language Pathologist

About the role

Join our caring community as a Home Health Speech Language Pathologist, reporting to the Clinical Manager and delivering personalized rehabilitation services in patients' homes or facilities.

Responsibilities

• Evaluating, directing, and providing speech, voice, language, hearing, and swallowing therapy
• Developing and reviewing treatment plans and care protocols
• Assessing and treating communication and related disorders using diagnostic tests and therapeutic interventions
• Monitoring patient progress, documenting outcomes, and reporting changes to the team
• Training patients and families on communication aids and strategies
• Participating in care coordination, discharge planning, and maintaining confidentiality

Requirements

• Minimum six months of experience as a speech therapist or speech-language pathologist
• Current, unrestricted license and CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• TB screening and driver safety compliance

Preferred Qualifications

• Home health experience
