Responsibilities
- Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
- Develop and periodically review the Plan of Treatment and Plan of Care.
- Assess and treat disorders of speech, voice, language, hearing and swallowing.
- Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
- Record and report changes in the patient's condition and response to treatment.
- Provide instruction and training to patients in use of alternative communication systems.
- Provide counseling and instruction to patients, families and healthcare staff.
- Maintain and submit documentation as required by the Company and/or facility.
- Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports.
- Participate in care coordination activities and discharge planning.
- Maintain the highest standards of professional conduct concerning information that is confidential in nature.
Requirements
- Minimum six months experience as a speech therapist / speech language pathologist.
- Current and unrestricted license.
- CPR certification.
- A valid driver's license, auto insurance, and reliable transportation are required.
Preferred Qualifications
- Home Health experience.
Pay
Pay Range: $54.00 - $76.00 pay per visit/unit
Base pay: $85,400 - $117,500 per year

- TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
- Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
