About the Role

The Home Health Nurse plans, organizes, and directs home care and hospice services with an emphasis on community health education. This role builds community resources to meet the needs of individuals and families within their homes and communities.

Responsibilities

• Completes initial and ongoing assessments of patient and family needs to determine home care requirements.
• Provides a complete physical assessment and medical history of current and previous illnesses.
• Regularly re-evaluates patient nursing needs and adjusts the plan of care as needed.
• Develops and revises care plans based on nursing diagnoses, patient status, and goals, incorporating therapeutic, preventive, and rehabilitative actions.
• Includes the patient and family in care planning and goal-setting.
• Administers medications and treatments as prescribed by the physician.
• Provides direct patient care in accordance with the State Nurse Practice Act.
• Counsels patients and families on nursing and related needs.
• Provides health care instructions tailored to the patient’s assessment and plan of care.
• Identifies and implements discharge planning needs as part of the care plan.
• Acts as Case Manager when assigned, coordinating patient care for an assigned caseload.
• Prepares clinical notes and updates the primary physician at least every 60 days or as necessary.
• Communicates with physicians regarding patient needs, changes in condition, and obtains physician orders as required.
• Coordinates care plans with community health-related professionals.
• Participates in on-call and holiday duties.
• Ensures availability of necessary equipment and services.
• Instructs, supervises, and evaluates home health aide care in compliance with state and federal regulations.

Requirements

• Graduate of an accredited school of nursing.
• One to two years of acute care experience in an institutional setting preferred.
• Current state licensure and CPR certification.
• Compliance with acceptable professional standards and practices.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Proficient in nursing skills per competency checklist.
• Ability to stand, walk, lift, position, or transfer patients, and lift supplies and equipment.
• Capable of reaching, stooping, bending, kneeling, or crouching.
• Visual acuity and hearing sufficient to perform required nursing skills.
• May occasionally lift and/or carry items weighing up to 50 pounds.
• Licensed driver with an insured automobile in good working order.
• Knowledge of potential exposure to communicable diseases and bodily fluids.
• Ability to work under stress in emergency situations during irregular hours.
