About the role
Providing specialized nursing care in patients' homes within the Southeast Region.
Responsibilities
- Assess patient needs, develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure quality, evidence-based care
- Promote cultural sensitivity and advocate for patients
- Evaluate changes in condition and update care plans accordingly
- Provide comprehensive home health services, including skilled nursing, hospice, rehabilitation, and specialized programs
- Participate in on-call rotations, weekend and holiday coverage
- Utilize provided technology tools
Qualifications
- Valid RN license
- Nursing Diploma or Associate's degree (Bachelor's preferred)
- Valid Driver's License
- Less than 2 years experience required; 2-5 years preferred
- Must be willing to provide coverage across all regions during on-call rotations
