Home Health RN--North Shore

About the role

Joining the BILH team means making a difference in people's lives. This Home Health position offers condensed territory coverage and only one weekend rotation every two months.

Responsibilities

• Completes and documents an initial assessment of patient and family to determine home care needs.
• Initiates and revises the plan of care as needed.
• Administers medications and treatments as prescribed by the physician.
• Instructs, supervises, and evaluates the care provided by home health aides every two weeks.
• Identifies discharge planning needs as part of the care plan development.
• Practices confidentiality according to agency and Federal HIPAA guidelines.
• Demonstrates proficiency in OASIS completion and completes documentation through EPIC in accordance with agency policies.

Requirements

• An active valid Massachusetts RN license.
• An active American Health Association BLS certification.
• A minimum of 1 year of direct care nursing experience, preferably in public health or home care nursing.
• An Associate’s Degree or BSN (preferred).
• OASIS and/or EPIC experience is preferred.

Pay

$79,000.00 USD - $114,900.00 USD

Schedule

Flexible schedule with only one weekend rotation every two months.
