About the Role

We are seeking a Full-Time Home Health RN Clinical Manager.

Responsibilities

• Experience: Five (5) years of Home Health experience (Medicare knowledge a must)
• Management Experience: At least three (3) to five (5) years in a RN clinical management role
• Knowledge: Working knowledge of SHPMust be clinically sound and able provide training to new clinicians in the community.
• Understanding: Understands VBP and PDGM impact on home health servicesExtensive OASIS knowledgeAn understanding of Intake and Scheduling processes
• Supplies Management: Strong knowledge of wound care a plusWilling to assist with Scheduling, Clinical Issues, and Patient Visits
• On-call Availability: Able to manage on-call at least once per month

Requirements

• System Knowledge: Must be system savvy (able to toggle between multiple screens and systems simultaneously) Matrix Care (Preferred) MS Office (Excel, MS Teams, Outlook, etc.)

Pay

$95,000 - $100,000 per year
