Description
The Home Health RN Care Coordinator at St. Luke’s will work with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Responsibilities include planning, implementing, and documenting nursing care based on patient needs, and ensuring the patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records.
Requirements
- Nursing Degree
- 2 years of RN experience
- Current RN License in the State of Practice
- Valid Driver's License and Auto Insurance if work related travel is required for position
- Current Basic Life Support (BLS) Provider Certification
Qualifications
- Reliable personal transportation during work hours with current auto registration and auto insurance
- Hours: Monday - Friday (8:30am - 5:00pm) with rotating/variable weekends including rotating/variable weekday and weekend daytime/evening call up to 8:00 pm
