Job Duties

• Skilled Visits to patients based on their needs
• Daily patient interaction, ensuring comfort and support
• Education on health conditions including, but not limited to, CHF, COPD, Diabetes
• Wound care and assessments
• Medication management and administration
• Change dressings, bandages, and other contraptions as prescribed
• Documentation of patient's condition, reporting any significant changes to the physician
• Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists

Requirements

• Active RN Nursing License in the state
• Ability to work in a fast-paced, dynamic environment
• Excellent organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver's license and reliable transportation
