Home Health Registered Nurse - Transition Team RN

About the role

Seeking Registered Nurses (RNs) to join the Transition Team, a specialized unit providing immediate in-home care to support patients transitioning back home.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process
• Develop and update care plans, requesting provider revisions as needed
• Administer medications, treatments, and respond to patient condition changes
• Provide therapeutic, preventive, and rehabilitative interventions
• Educate patients and families, coordinate care, and assist with discharge planning

Qualifications & Skills

• Current RN license in practice state
• Valid health certificate, BLS certification, and PPD or Chest X-Ray
• Ability to travel up to an hour and care for pediatric and adult patients
• Experience with varied acuity patients (training available); 1+ year RN experience preferred
