Home Health Registered Nurse / RN (Weekends)

Mission Healthcare, the largest home health and hospice company in the western United States, is seeking a Registered Nurse to provide comprehensive in‑home care in North San Diego County. Responsibilities

• Complete an initial assessment of the patient and family to determine home care needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Initiate and revise the plan of care as patient status and needs change.
• Regularly re‑evaluate patients’ needs and use health assessment data to determine nursing diagnoses.
• Develop a care plan with goals based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative actions, and include the patient and family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by the physician.
• Counsel the patient and family in meeting the plan of care and related needs.
• Provide health‑care instructions to the patient as appropriate per assessment and plan.
• Identify discharge‑planning needs and implement them prior to patient discharge.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute‑care experience in an institutional setting.
• Basic Life Support (BLS) certification with hands‑on CPR.
• Valid driver’s license and auto insurance.
• Associate’s or Bachelor’s degree in Nursing.
• One year of home‑health experience preferred.

Pay

$60.00‑$70.00 hourly, depending on experience.

Schedule

Part‑Time, Friday – Monday, 8:00 am – 5:00 pm. Territory: North San Diego County.
