Home Health Registered Nurse (RN) - Part & Full Time ($3-$5K Bonus)

About the role

We are seeking dedicated and compassionate Registered Nurses (RNs) to join our team as home health/home care RNs. Positions are available in full-time, part-time, and contingent capacities.

• Full-Time RN: 30 points per week
• Part-Time RN: 18 points per week
• Contingent RN: Flexible hours with opportunity to transition into full-time or part-time caseload

Responsibilities

• Provide care to patients in the community to help them regain independence.
• Conduct and complete initial assessment of patients to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Observe signs and symptoms and report to the physician and/or other appropriate health professionals as needed, or upon changes in the patient’s condition.
• Teach, supervise, and counsel the patient and family regarding home nursing procedures and other care needs as appropriate.
• Exhibit critical thinking and apply it for continuous improvement of services and the agency.

Requirements

• Active Michigan Registered Nurse (RN) license
• Reliable transportation
• Valid driver’s license and auto insurance in your name as a driver
• Home health care experience preferred

Pay

• Full-Time RN: $60.00 - $100.20 per visit (approximately $93,600/year with full-time points)
• Part-Time RN: $60.00 - $100.20 per visit (approximately $56,160/year with part-time points)
• Contingent RN:
  • Weekday Visit Rate: $60.00 - $100.00 per visit
  • Weekend Visit Rate: $65.00 - $105.00 per visit
• Additional Reimbursement:
  • Lab Drop Off: $15.00 per lab drop off
  • Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge
  • Transfer Without a Visit: $10.00 per non-visit transfer
  • Orientation Period and In Services: $30.00 per hour
  • Weekend On Call: $100.00 per weekend day or $200.00 per regular two-day weekend
  • Mileage Reimbursement: $0.50 per mile after 30 miles per day round trip from your home
