About the role
This is NOT a remote position.
Responsibilities
- Makes rounds to monitor patient's overall health
- Develops an individualized plan of care with a primary care physician
- Works independently to deliver quality healthcare to patients
- Provides specialized services such as wound therapy, post-surgical care management, and ostomy care management
- Handles tasks like pre-populating medication reconciliation, tracking DME orders, getting MD signatures, coding, and lab follow-up
- Functions as a case manager
Requirements
- Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
- Active CA Registered Nursing License
- AAS, BSN, MSN degree
- Reliable transportation
- Current CPR or BLS certification

Pay
Compensation is based on a full-time schedule.
