Home Health Registered Nurse RN - Full Time - 10K Bonus

About the Role

Classic Home Care, a nurse-owned and operated skilled home health agency, has been proudly serving communities across Southeast Michigan since 2003. Our office is located in Northville, Michigan, and we provide care to patients in many surrounding areas. We are Joint Commission-accredited and Medicare-certified. We are seeking dedicated and compassionate Registered Nurses (RNs) to join our team as home health / home care RNs.

Responsibilities

• Provide care to patients in the community to help them get back to their life of independence.
• Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Observe signs and symptoms and report to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition.
• Teach, supervise and counsel the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition.
• Exhibit critical thinking and apply them for continuous improvement of services and the agency.

Requirements

• Active Michigan Registered Nurse (RN) license
• Reliable transportation
• Valid driver's license and auto insurance in your name as a driver
• Home health care experience preferred

Compensation & Benefits

Pay: $60.00 - $100.20 per visit (approximately $93,600/year with full-time points).

Additional reimbursements:

• Lab Drop Off: $15.00 per lab drop off
• Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge
• Transfer Without a Visit: $10.00 per non-visit transfer
• Orientation Period and In Services: $30.00 per hour
• Weekend On Call: $100.00 per weekend day or $200.00 per regular two day weekend

Mileage Reimbursement:

• Business Days (Monday-Friday): $0.50 per mile after 30 miles per day round trip from your home back to home
• On-Call Weekends & Holidays: $0.50 per mile round trip from your home back to home (no 30-mile deduction)

Schedule & Travel

Full-time hours (30 points per week). Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
