Stella Maris provides compassionate, skilled home health care for adults in central Maryland, serving Baltimore City, Baltimore County, Harford County and Howard County. They are seeking a Home Health Registered Nurse (RN) to join their growing Home Health team in Baltimore County. This full‑time position offers the opportunity to work alongside dedicated clinicians to meet community needs. Responsibilities

• Perform primary nursing care functions under the direction of the clinical manager, conforming to professional standards of home health nursing practice.
• Provide individualized care to patients in their home according to the physician‑approved plan of care.
• Coordinate all services for each assigned patient and family.
• Provide leadership and advocacy in coordinating patient care with multiple clinicians, modalities, and community resources.
• Manage clinical resources and transition planning for patients within the assigned caseload from pre‑admission through post‑discharge.
• Explain home care and hospice services to prospective patients/families accurately, thoroughly, and in an understandable manner.
• Explain insurance benefits, coverage, patient responsibilities, and alternative payment options.
• Develop the initial plan of care based on input from patient, family, and physician.
• Request appropriate disciplines for care.
• Report admissions to the Patient Care Coordinator within 24 hours and update the Hospice Team at weekly team meetings.
• Document accurately all admissions, follow‑up visits, and phone calls.
• Review charts for completeness and accuracy when a case is closed.
• Perform daily travel in the West Baltimore County, Maryland region.

Requirements

• RN licensure in the State of Maryland.
• Minimum 2 years experience in home health.
• OASIS Documentation experience required.
• EPIC experience preferred.
• Valid driver’s license and reliable transportation.

Pay

Minimum USD $38.05 per hour; Maximum USD $57.05 per hour. Compensation decisions align with internal equity, role scope, and market data; most new hires are placed in the lower to mid‑portion of the range, with higher offers reserved for candidates with advanced or highly specialized qualifications.

Schedule

Full‑time, Monday‑Friday 8:00 a.m. – 4:30 p.m.
