About the role

The Dulla Care Home Healthcare RN serves as a key contributor in bringing individualized care to clients' homes as an alternative to hospitalization. This role involves providing direct and in-direct nursing care, supervising home care staff, managing lab draws, IV insertion, and medication administration. The RN engages clients in their healthcare journey, establishing meaningful connections, and implementing a multidisciplinary plan of care.

Responsibilities

  • Provides direct and in-direct nursing care to clients in home care settings; to include admission framework.
  • Home care staff supervision and oversight; appropriation of discrepancies as warranted.
  • Labs draws, IV insertion, medication administration.
  • Engage clients in their healthcare journey and provide meaningful tailored education.
  • Establish meaningful connections and trusting relationships with each client.
  • Appropriate charting, noting in OASIS and/or medical records.
  • Implements multidisciplinary plan of care for clients in Dulla Care Home Healthcare program services.
  • Works closely with the Dulla Care Home Healthcare leadership team to support and promote the care redesign of Dulla Care Home Healthcare program services.
  • Oversees the planning, scheduling, and implementation of day-to-day clinical activities for clients.
  • Ensures appropriate DME and services are ordered and initiated in a timely manner.
  • Acts as a central point of contact for all navigated clients; including interaction with medical, clinical and care staff.
  • Collaborates with medical providers, client care staff and skilled services in the planning and implementation of client and education plans.
  • Participates in client education and engagement; ensures educational goals are on track, actively assess the client for understanding.
  • Maintains clients’ daily compliance with the plan of care and tracks order completion and follow-up, engaging with the client throughout the episode of care.
  • Aid clients and care partners with referrals to community resources and care coordination that can assist with the social determinants of health as needed.
  • Makes daily EVV check-ins and out.

Requirements

  • BSN preferred.
  • Certification/Licensure in the State of Ohio as a Licensed Registered Nurse or licensure from a state participating in the multistate privilege to practice compact with Ohio.
  • Minimum of 7 years acute care or home care nursing experience.
  • BLS and ACLS Certified.

  • Reach and grasp with arms and hands, including reaching overhead/above shoulder level, below the waist, and in front of body.
  • Move within confined spaces.
  • Maintain physical tolerance and balance in multiple positions.
  • Stand (at client's side during treatment, etc.).
  • Sustain repetitive movement (CPR compressions, bagging, etc.).
  • Valid driver’s license with clean driving record.
