Home Health Registered Nurse PRN

About the Role

Serve as the primary clinical partner for patients and families, providing individualized nursing care in the home to promote recovery, safety, and independence.

Responsibilities

• Conduct comprehensive in-home assessments and develop patient-centered care plans.
• Deliver skilled interventions such as wound care, infusions, medication management, and chronic disease management.
• Educate and empower patients and caregivers, monitor patient progress, and identify early changes in condition.
• Communicate effectively with the care team to adjust plans and prevent hospitalizations.
• Coordinate care across providers and ensure continuity.
• Complete accurate documentation supporting quality and compliance.

Requirements

• Active Registered Nurse (RN) license in the practice state.
• At least one year of home health nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Reliable transportation and a valid driver’s license.
• CPR certification or ability to obtain it.
