Responsibilities
- Manage your own caseload of home health patients
- Coordinate interdisciplinary care with physicians and other team members
- Develop and update individualized care plans
- Conduct comprehensive in-home assessments and provide patient education
- Coordinate community resources, specialists, therapy services, and family meetings
- Monitor medication safety and patient compliance
- Complete accurate clinical documentation and ensure regulatory compliance
- Prevent rehospitalizations through proactive care management
Requirements
- Active Connecticut RN license
- Graduate of an accredited nursing program (BSN preferred)
- Minimum 1 year of hospital or acute care nursing experience
- Valid driver's license and reliable transportation
- CPR Certification
- Computer proficiency with electronic medical records
Qualifications
- Independently manage a home health caseload from admission to discharge
- Develop and update individualized care plans with interdisciplinary teams
- Perform comprehensive in-home assessments and patient education
- Coordinate community resources, specialists, therapy services, and family meetings
- Monitor medication safety and patient compliance
- Complete accurate clinical documentation and regulatory compliance requirements
Skills
- Strong clinical assessment, documentation, and care coordination skills
- Independent case management experience
- Home Health or Visiting Nurse experience
- OASIS documentation experience
- Medicare/Medicaid regulations knowledge
- Case management experience
- CHAP or Joint Commission experience
- Experience working with interdisciplinary care teams
- Strong patient education and communication skills
