Home Health Registered Nurse - Full Time

About the Role

We are seeking a dedicated Registered Nurse (RN) for full-time home health care in the Appleton, Neenah, Menasha, and New London areas.

Responsibilities

• Provide personalized, one-on-one nursing care in patients’ homes to promote recovery, safety, and independence.
• Conduct comprehensive in-home assessments to develop patient-centered care plans.
• Administer skilled interventions such as wound care, infusions, medication management, and post-operative care.
• Educate and empower patients and caregivers to manage symptoms and treatments confidently.
• Monitor patient progress, identify early changes, and coordinate with healthcare teams to adapt plans and prevent hospitalizations.
• Ensure seamless care coordination and maintain accurate, timely documentation to support quality outcomes and compliance.

Requirements

• Active RN license with at least 1 year of clinical experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Reliable transportation, valid driver’s license, and CPR certification (or ability to obtain).
