Home Health Registered Nurse

Responsibilities

• Perform in-home skilled nursing visits - SOCs, recerts, routine skilled visits, discharges;
• Complete thorough assessments, implement plan of care, and educate patients/families;
• Document accurately and on time in the EMR (assessments, care provided, orders, signatures) to support billing, compliance, and continuity;
• Care coordination & communication with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current;
• Monitor progress & escalate concerns (changes in condition, safety risks, non-compliance) to prevent adverse events and avoid unnecessary hospitalizations;
• Support compliance & quality - follow policies, payer requirements, and home health standards; participate in QA and care plan updates.

Requirements

• 3+ years' nursing experience;
• Active Florida RN license;
• Reliable transportation for in-home visits;
• Prior home health/field experience (SOC/Recert/Discharge experience preferred);
• Strong documentation & OASIS skills;
• Spanish speaking (preferred).

Pay

$1,500-$2,000 weekly.

Schedule

Visits fall within business hours (8:00 AM–8:00 PM); you won't work all 12 hours.
