Wheatland Health Community Care is hiring Home Care RNs across Osceola County and Brevard County, FL for Full-Time or Part-Time roles. You'll deliver skilled, compassionate care that protects independence at home—and you'll feel the rewards every single day.

Responsibilities

• Map your visits within your chosen territory
• Complete SOCs, recerts, routine skilled visits, and discharges
• Perform thorough assessments, implement the plan of care, and teach patients and caregivers in language that sticks
• Capture accurate EMR documentation (assessments, care, orders, signatures)
• Respond quickly to changes in condition, safety concerns, or non-compliance to prevent avoidable ED trips
• Review QA insights and ensure policies and payer rules are met

Requirements

• Active Florida RN license and reliable transportation
• 3+ years of nursing experience
• PREFERRED: Home health or field-based experience
• SOC/recert/discharge experience
• Strong documentation & OASIS
• Spanish a plus

Pay

$1,500-$2,000 weekly

Schedule

Full-Time or Part-Time with visits scheduled within 8:00 AM–8:00 PM; choose at least one territory and cover all ZIP codes in that territory (you may choose multiple)
