Responsibilities
- Perform in-home skilled nursing visits - SOCs, recerts, routine skilled visits, discharges
- Complete thorough assessments, implement plan of care, and educate patients/families
- Document accurately and on time in the EMR (assessments, care provided, orders, signatures)
- Coordinate care & communicate with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current
- Monitor progress & escalate concerns (changes in condition, safety risks, non-compliance) to prevent adverse events and avoid unnecessary hospitalizations
- Support compliance & quality - follow policies, payer requirements, and home health standards; participate in QA and care plan updates
Requirements
- 3+ years' nursing experience
- Active Florida RN license
- Reliable transportation for in-home visits
- Prior home health/field experience (SOC/Recert/Discharge experience preferred)
- Strong documentation & OASIS skills
- Spanish speaking (preferred)

Pay
- $1,500-$2,000 weekly
- Additional per-visit rate of approximately $65–$70 for visits completed above the productivity expectation
Schedule
- Visits fall within business hours (8:00 AM–8:00 PM)
- You won't work all 12 hours
