Home Health Registered Nurse

About the role

Join a compassionate home health team as a Full-Time Registered Nurse (RN) and make a difference in patients' lives by providing personalized care in their homes.

Responsibilities

• Manage individualized Plans of Care to promote safety and independence at home.
• Provide skilled nursing services such as medication administration, wound care, and patient education.
• Perform assessments and update care plans based on patient progress.
• Act as the primary liaison between physicians, patients, families, and the care team.
• Coordinate care with multidisciplinary team members and educate patients on health management and safety.
• Communicate patient updates to ensure appropriate interventions.
• Support quality initiatives with accurate documentation.

Requirements

• Active RN license in the state of employment.
• At least one year of high acuity nursing experience.
• Compassionate, flexible, and patient-centered approach.
• Excellent organizational, communication, and clinical decision-making skills.
• Valid driver’s license, reliable transportation, and current CPR certification.

Preferred Experience

• Background in home health, hospice, ICU, geriatrics, Med-Surg, ER, telemetry, or related clinical settings.
