About the role

The role involves providing comprehensive home health nursing care across all age groups, including adults, geriatrics, and pediatrics, by utilizing the nursing process (assess, plan, implement, evaluate).

Responsibilities

• Assess, interpret, plan, and evaluate patient care based on age and diagnosis.
• Manage a caseload efficiently, coordinating with a multidisciplinary team.
• Communicate effectively following agency policies and guidelines.
• Contribute to program effectiveness and maintain a positive work environment.
• Organize work, adjust schedules, and demonstrate commitment to agency values.
• Promote customer satisfaction and uphold the mission, vision, and values.

Qualifications

• Graduate of an accredited nursing program (AD, Diploma, BSN).
• Current nursing license in the relevant state and CPR certification.
• Minimum of 1 year RN experience and a total of 2+ years clinical experience; home health preferred.

Skills

• Basic technical skills, strong interpersonal abilities, and physical capability to perform patient care tasks.
• Ability to assess patients, develop care plans, and educate patients effectively.
• Visual and auditory acuity for patient assessment and communication.
• Must operate a vehicle and hold a valid driver’s license.

Requirements

• Exposure to potential hazards such as bloodborne pathogens and environmental risks.
• Ability to handle physical demands like lifting and transferring patients.

Schedule

• Flexible work hours, including weekends and holidays, as needed.
