Home Health Registered Nurse

As a Home Health Registered Nurse, you'll deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own environment. This role is ideal for an RN who values clinical independence, critical thinking, and building strong relationships with patients and families over time.

Responsibilities

• Manage a caseload of patients from admission through discharge
• Provide skilled nursing care
• Conduct assessments
• Develop individualized care plans tailored to each patient's needs
• Coordinate services with physicians and an interdisciplinary care team
• Make informed clinical decisions
• Play a central role in shaping patient outcomes and supporting each person's journey toward greater health and independence

Requirements

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum 1 year of experience as a licensed RN in a clinical care setting
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Qualifications

• Bachelor of Science in Nursing (BSN)
• Experience in home health environment
• OASIS certification or willingness to obtain

• Proficiency in OASIS documentation and care planning
• Excellent time management and organizational abilities
• Effective communication and teaching skills
• Ability to work independently with minimal supervision
• Comfort working in diverse home and facility environments

• Understanding of Medicare, Medicaid, and third-party payor requirements
