Responsibilities

• Conduct assessments and develop/update individualized care plans
• Administer medications and treatments as prescribed
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Ensure regulatory compliance and thorough documentation
• Deliver high-quality, patient-centered care in a home setting

Qualifications & Skills

• Current valid RN license
• Strong clinical assessment and care planning skills
• Excellent communication and coordination abilities
• Knowledge of healthcare regulations and documentation standards
