Home Health Registered Nurse

About the role

The home health registered nurse provides comprehensive patient care in the home setting, assessing, planning, implementing, and evaluating care across all age groups, including adults, geriatrics, and pediatrics.

Responsibilities

• Assess, interpret, and develop individualized care plans based on patient diagnosis and age.
• Manage a caseload efficiently, coordinating with multidisciplinary teams to ensure quality care.
• Communicate effectively following agency policies and maintain positive relationships with patients, families, and staff.
• Contribute to program effectiveness and promote patient satisfaction.
• Organize work schedules to optimize agency performance and demonstrate commitment to organizational values.

Qualifications

• Graduate of an accredited nursing program with a current state license (NC/SC) and CPR certification.
• Minimum of one year RN experience, with additional clinical or home health experience preferred.

Skills

• Strong interpersonal, communication, and physical skills, including the ability to lift objects and maneuver patients.
• Ability to assess patient conditions, develop care plans, and educate patients effectively.
• Must meet sensory and mental requirements for safe patient care, including visual and auditory acuity, problem‑solving, and prioritization skills.

Requirements

• The role involves exposure to various hazards and requires a valid driver’s license and operational vehicle.
• Flexibility is needed to work variable hours, including weekends and holidays, serving diverse populations across different age groups.
