Home Health Registered Nurse
About the Role
As a CenterWell Home Health nurse, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. A $10,000 sign-on bonus is available for this role.
Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients' conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient's physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
Requirements
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver's license, auto insurance, and reliable transportation.
- Med surg, ICU, ER, or acute experience.
Preferred Qualifications
- One year of nursing experience.
- Home Health experience.
Pay
The pay range for this role is $49.00 - $69.00 per visit/unit. The starting base pay range for full-time employment (40 hours per week) is $77,200 - $106,200 per year, which may vary based on geographic location and individual qualifications such as skills, knowledge, experience, education, and certifications.
Schedule
Scheduled weekly hours: 40.

Additional Information
- This role is considered patient-facing and requires Tuberculosis (TB) screening.
- This role requires a valid state driver's license and maintenance of personal vehicle liability insurance. Minimum required limits are $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, or higher if required by the residing state.
