Responsibilities
- Assess patient needs and develop/update personalized care plans
- Administer treatments and monitor patient progress
- Educate patients and caregivers on health management
- Support recovery, chronic disease management, and hospice care
- Coordinate with families and an interprofessional care team
- Adapt to changing assignments as a float pool nurse, serving diverse communities
Qualifications
- Current Wisconsin RN license
- Reliable transportation
- Relevant nursing experience preferred
