Home Health Registered Nurse

About the Role

As a Home Health Nurse, you will provide personalized, skilled nursing care in patients' homes, collaborating with healthcare professionals to develop and implement care plans that support recovery and independence.

Responsibilities

• Administer medications, perform wound care, and conduct patient assessments onsite.
• Develop and adjust individualized care plans in partnership with physicians and clinicians.
• Monitor patient conditions, report changes, and educate families on disease management and treatments.
• Maintain accurate records and coordinate with healthcare team members.
• Supervise and oversee Home Health Aides, ensuring proper documentation completion.

Requirements

• Experience in Med-Surg, ICU, ER, or acute care settings.
• Current RN licensure with unrestricted status.
• CPR certification and valid driver’s license with reliable transportation.
• Ability to collaborate effectively with healthcare teams.
• TB screening and driver safety compliance.
