Home Health Registered Nurse

Responsibilities

• Provide one-on-one nursing care in the comfort of patients' homes
• Complete assessments, develop care plans, and deliver high-quality clinical care
• Educate patients and families to support independence and confidence
• Collaborate with physicians, therapists, and your care team to ensure seamless care
• Document visits accurately and timely using our user-friendly EMR
• Support patients through recovery, chronic condition management, and health maintenance

Requirements

• Active RN license
• Active BLS certification
• Active CPR certification
• Experience with OASIS-based home health documentation
• Experience with care planning, medication management, and coordination of care
• Experience working with patients and families, physicians, therapists, and the full care team
• Comfort with technology and electronic documentation systems
• Eligible to work in the United States
• Reliable transportation
• Home health experience is a plus, but not required — we're happy to train the right nurse
• Bachelor of Science in Nursing (BSN) is a plus
• Experience with Kinnser is a plus
