Responsibilities

  • Perform comprehensive physical therapy evaluations in patients' homes
  • Develop individualized plans of care based on each patient's functional needs, goals, and medical condition
  • Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, gait, and overall independence
  • Establish measurable goals and continually assess patient progress
  • Modify treatment plans as clinically appropriate
  • Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety
  • Assess the need for assistive devices, durable medical equipment, and home safety modifications
  • Coordinate and oversee care provided by Physical Therapist Assistants when applicable
  • Communicate patient progress and changes in condition with physicians and the interdisciplinary care team
  • Complete timely and accurate clinical documentation in accordance with Medicare, regulatory, and agency requirements
  • Collaborate with nurses, therapists, physicians, and other healthcare professionals to provide exceptional patient-centered care

Requirements

  • Graduate of an accredited Physical Therapy program
  • Current and unrestricted Arizona Physical Therapist license
  • CPR Certification (or ability to obtain)
  • Valid driver's license
  • Reliable transportation and current auto insurance
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently while collaborating effectively with an interdisciplinary team

Home health experience is preferred but not required. New graduates are encouraged to apply.
