Home Health Physical Therapist PRN

About the role

We are hiring a resident-centered PRN Home Health Physical Therapist to join our team at Erickson Senior Living's Charlestown campus. Our residents live within our award-winning campus, allowing you to be part of their journey throughout their continuum of care.

Responsibilities

• Implement residents’ personal care plans, focusing on residents' strengths, preferences, and preferred routines.
• Support the physical, spiritual, emotional, and clinical needs of residents.
• Work closely with the interdisciplinary clinical team to address the needs of our residents.
• Provide wellness visits and sick care for all employees over 18 years of age.

Requirements

• Must possess a valid and current state Physical Therapy license.
• Training must meet current Medicare and state requirements.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Qualifications

• Experience in home health care is required.
• Must meet current Medicare and state requirements for licensure and training.

Skills

• Strong interpersonal skills to support residents' physical, emotional, and spiritual needs.
• Ability to document care effectively to meet reimbursement and regulatory requirements.
• Knowledge of therapeutic techniques for older adults.

Pay

$45.00 per hour.
