Home Health Physical Therapist-PRN
About the role
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. Responsibilities
- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
Preferred Qualifications
- One year of experience as a Physical Therapist
Pay
- Pay Per Visit/Unit Rate: $59.00 - $83.00
- Full time (40 hours per week) base pay range: $93,000 - $128,000 per year (may vary based on geographic location and individual factors such as skills, knowledge, experience, education, certifications, etc.)

Additional Requirements
- Tuberculosis (TB) Screening: This role is considered patient facing and is part of Humana's TB screening program. If selected, you will be required to be screened for TB.
- Driving: This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
