Home Health Physical Therapist-Per Diem
About the role
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
Responsibilities
- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
Preferred Qualifications
- One year of experience as a Physical Therapist.
Pay
Per Visit/Unit Rate: $71.00 - $99.00
Full-time (40 hours per week) starting base pay range: $111,600 - $153,600 per year. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, and certifications.

Additional Information
- This role is considered patient-facing and requires Tuberculosis (TB) screening.
- This role requires a valid state driver's license and personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
