Position Overview

Aveanna Healthcare is seeking a Physical Therapist (PT) to join our dynamic home health care team. This role enables patients to recover from illness or injury in the comfort of their own home.

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Develop and implement a written evaluation and plan of care that defines physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services according to prescribed physician orders.
• Document client performance, modify goals and treatment plans as necessary, and develop discharge care plans as required.
• Collaborate with healthcare teams, physicians, parents/guardians, nurses, and others to ensure comprehensive care.
• Ensure accurate and timely documentation to avoid payer denials.
• Maintain effective communication with staff, healthcare team members, and families, serving as a resource consultant for clinical staff.
• Supervise other personnel (PTAs, HHAs) as applicable.
• Complete OASIS assessments where permitted by state regulations.

Requirements

• Master's degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.

• Experience in Home Health Therapy and completion of OASIS documentation.
