About the role
This position is a contributing member of the interdisciplinary team and evaluates and services to the home health patient with neurological, musculoskeletal, cardiopulmonary impairments; gross motor, sensorimotor, and developmental delays; as well as traumatic injuries and orthopedic disorders.
Responsibilities
- Provide care and services for a group of patients, visiting patients where they reside, which could include private homes, Independent Living Facilities, and Assisted Living Facilities.
- Evaluate and service patients with neurological, musculoskeletal, cardiopulmonary impairments; gross motor, sensorimotor, and developmental delays; as well as traumatic injuries and orthopedic disorders.
- Collaborate daily with a team of clinicians.
- Work closely with patients on a 1-1 level but get strong support from a patient care team.
- Work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations.
- Manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns).
- Manage time effectively to prioritize and address multiple concerns.
Requirements
- Current Physical Therapist licensure (in good standing) in the state(s) of practice with one year of experience under license.
- Ability to work well within a team environment to collaborate daily with a team of clinicians.
- Work closely with patients on a 1-1 level but get strong support from a patient care team.

Qualifications
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
- CPR Certification.
