Home Health Physical Therapist
Duties
- Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines.
- Develop and implement individualized treatment plans in collaboration with physicians.
- Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.
- Monitor and document patient progress, reporting changes to physicians and nurse supervisors.
- Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.
Requirements
- Education: Experience in home health or related clinical settings preferred.
- Licenses: Active Physical Therapist license.
- Certifications: Current CPR certification.
- Years of experience: 1 year

Pay
Pay Range: $50 - $56 on w2/Pay Rate is based on experience and educational qualifications. *Pay offered is based on experience, expertise, credentialing, and education.
